Where are you located?

Our office is at 3547 Hendricks Ave, in between Parkwood street and Sheridan street.  There is street parking right in front of our office, parking behind the office is for employees only.  

How long are appointments?

The length of the appointment depends on what the appointment is for.  Simple medication managment follow-ups last between 15-30 minutes, while new client psychiatric evaluations last between an hour to an hour and 30 minutes.  Plan to be at the office for 90 minutes for a ketamine infusion.

How do I make an appointment?

The quickest way is through our website.  Just click on the "Book Online" button located at the top of this page.  

If you're on a mobile device, click on the hamburger menu (3 lines in the upper left hand corner of the screen) and click the "Boonk Online" button.

You can also call the office and have a team member schedule your appointment.

How will my visit differ if I see a Psychiatrist or a Advanced Practitioner?

Your appointment with the MD or Nurse Practitioner will be conducted the same and you will receive the same gold standard of care. Both can write the same prescriptions and follow the same guidelines.

What age range does The Practice serve?

We offer psychiatric care to ages 5 and up.

How do I get my medications refilled?

Refills are only written during regularly schedule appointments.

Do you offer telehealth appointments?

Yes we do!  Your first appointment must be in person, but after that unless it is an appointment for which you have to be face to face such as ketamine infusions, you may elect the telehealth option.  

Clients who are on scheduled 3 or higher medications may be required to have in office appointments, this included medications such as Adderall, Xanax, Clonazepam, etc.

How often are follow-up appointments?

Being an outpatient practice we do not provide inpatient or emergency services. Appointment frequency varies depending on your condition. The most common range is every 1-3 months. This allows time for treatment to take full effect and assess your response.

Appointments will be more frequent in the first several months of treatment. If you need more frequent or intense level of treatment we can refer you to services providing a higher level of care.

How do I prepare for my first appointment?

Before your first appointment, you will be sent a patient intake/consent forms packet via text and email, you may complete these from the comfort of your home, electronically. Completing your forms electronically streamlines your check-in the day of your appointment.

If you have a history of psychiatric treatment, ask prior treatment providers to send a confidential copy of your records to our office.

If you take any medications, ask your pharmacy to print out a list of your medication names and dosages for the last several years. FAX all of this information to (904) 877-1200.  Anyone under age 18 must be accompanied during the appointment by a parent or guardian.

How do I pay for my appointments?

We currently in-network with Aetna, Cigna, Humana, UnitedHealthcare, TRICARE, and straight Medicare. If you have a commercial plan from one of these carriers, and you have met your deductible for the year, you will only have to pay your co-pay, or co-insurance, according to your plan.

For those who have insurance through other carriers, or those who self-pay,  we offer very competitive rates, in fact many people who are insured by companies we are in-network with elect to self-pay thereby assuring their mental health remains just between them and their provider.
