New Patient Deposit: A deposit of $150 is required for all new patient appointments. This deposit will be applied to the cost of the first appointment. If the deposit exceeds copay, coinsurance, or deductible amounts, the balance will be applied to future visits.

Information will not be released to external parties without consent or a subpoena, except in cases of safety concerns or abuse/neglect issues.

Clients on controlled substance must have an appointment at least once every 2 months. All clients prescribed a controlled substance will be subject to urinary drug screens,

  6. Lost or Stolen Medications: In the unfortunate event that your medication is lost or stolen, please note that it will not be replaced. It is essential to safeguard your prescriptions and contact your provider promptly if such a situation occurs.

It is the policy of The Practice that a patient has to have a minimum of 1 year of attending regularly scheduled appointments with their provider before any disability will be considered.
